The Structured Chronic Disease Management (CDM) Programme aims to prevent and manage chronic diseases using a population-approach.

This programme is for patients who have a medical card or GP visit card and have a diagnosis of one or more of the listed conditions below.

• Type 2 diabetes
• Asthma
• Chronic obstructive pulmonary disease (COPD)
• Cardiovascular disease, including heart failure, heart attack (angina), stroke and irregular heartbeat (atrial fibrillation)

We offer a complete women's health service which may involve any of the following:

• Contraception - including long acting reversible contraception such as coils and implants.
• Combined Antenatal care - care during pregnancy
• Cervical Screening
• Breast care
• Menopause
• Bone Health and osteoporosis
• Fertility advice

We offer a complete men's health service which may involve any of the following:

• Prostate assessment
• Sexual health issues
• Vasectomy advice and referral
• Fertility advice
